Remember me poem- is there more and who wrote and what is the title?

we found the perfect gift for my mother a cross-stitch of this poem, but we can't find the rest, if there is any, of this poem. if you can tell us if there is more, what it is, where we can find the rest, who may have written it, what the title is, etc. PLEASE EMAIL ME!!! this is what we have so far:

Remember me with smiles and laughter For that's how I'll remember you all If you can only remember me with tears Don't remember me at all.
