Developing fault, has somebody seen?

Hi; one of my pupils developed (with an instructor) a roll of Agfapan 100 using Rodinal as usual. The result was something I haven't seen before during my 30 something years of film developing. it looks out like there was some foam in the tank, or the emulsion is defected or something.... Has somebody seen a fault like this? Any ideas what it could be? You know, the instructor is very experienced; all silly mistakes can be done by whoever, but not any beginners mistakes....
