PMK Pyro and Delta 400

Can someone please tell me what the mixing ratios are for parts A and B with water to make working solutions. How much of each working solution do you use per roll of 35mm? I have the liquid concentrates from Photographers Formulary. Also, do you reimerse the film in developer after fixing and if so, for how long. Thanks for taking the time to read and respond. Harvey
