Who chooses Patron Saints?

I have been unable to find an answer to the question, who it is who declairs a Saint a Patron Saint? How exactlly are they chossen? Any information would be helpful! Thank you and God Bless!

Most Patron Saints are "chosen" because of long-standing tradition and custom, ie, they have been venerated as that particular patron for time immemorial. But, there are also some patrons that are actually officially designated by the Pope. Often, the Patron of a country will have been officially designated, but patrons of professions, illnesses etc are from pious custom.
