Let's use this thread to collect instances of problems with federal government computer systems now that the feds have embarked on a new fiscal year (began today, Oct. 1). Be sure to post links/references when available.

I'll be anxious to see what types of failures you bloodhounds can dig up!

Boy, did we blow it bigtime on the Mars Climate Observer trajectory error. This kind of thing keeps me up nights. Fortunately, my project teams that have folks who attack basic problems like unit analysis right up front.
