Where do i go after i talk to the skull pillar ?

That depends on what you asked him, the portal will not open until you have asked the pillar at least 2 questions, 1. Where is the fortress of regrets, and 2. How do I leave this place. In both cases you will have to give up something. Give up the information about where Fjhull is and you either have to give up Morte or another member of your party, or give up 15 of your hit points by letting the pillar feed on your immortal blood.

Yes it is permanent. You have to gain them back one at a time.
