Response to knee-chondramalcia

Chondramalacia is typically referred to as a softening of the cartilage on the underside or back of the kneecap. It can be treated with a combination of medication and physical therapy, or as a last result surgery.

What would cause 'chondramalcia'. My mom had total knee replacement in July 2003 and only recently has had pain and swolleness around knee area. Doctors says atrophy, yet she has been active all the while up until this time with little problem. Does this make sense?
