There is an excellent chapter on The History of Professional Marriage and Family Therapy by Carlfred Broderick and Sandra S. Schrader in Alan S. Gurman & David P. Kniskern (Eds.), Handbook of Family Therapy (Brunner Mazel, 1981). It is repeated in Volume II (1991) of the Handbook. You'll find it interesting to see how interdisciplinary this field is.
