Alexander's first name was Alexander. It was also the last name he ever had.

Just because you and everyone you know has a first name, last name and most likely a middle name to boot, doesn't mean things worked like that in the fourth century BC. Back then people limped along with the one name and perhaps a nickname to help distinguish them from others with the same name. Like Black John, Loud Mikalis, Phillip One-Eye, or Alexander... the Great.
