Answers

Just a couple of remarks on that bibliography: Firstly, it is not complete, but gives an idea of what people have been writing about. Secondly this site runs lines together. Thus in the above bibliography, each element in the bibliography is on a separate line, but they come out all in one paragraph above. This makes the entries harder to read. However I hope it will serve some purpose.
