Who, within an organisation, would normally be responsible for issuing Database Standards and Guidelines for developing databases and documenting them and e.g. I.T.? Info Security?

Typically this responsibility would fall to the Database Administration group - and the DBAs would reside in the IT organization. Of course, the DBA group should work with other interested parties to ensure that the standards and procedures being developed are reasonable and proper. Interested parties may include system programmers/systems analysts, data administration, information security, application development, storage administration, and business intelligence professionals (to name a few).
